Family Health History Record

Why you should know your family’s history of cancer.

If members of your family have been diagnosed with cancer in the past, you might be at increased risk of developing cancer yourself. We encourage you to be aware of your family’s history of cancer and other conditions.

Knowing your risk can help reduce your chances of developing a disease or give you insights into when to get screened so we can detect cancer early. For most cancers, early detection increases your likelihood of more effective treatment and survival.

How to Create Your Family Health History

Download our Family Health History form (PDF) to collect your family’s information.

  • Talk directly to your relatives about any health conditions they’ve had, including chronic illness such as heart disease, miscarriages or other pregnancy complications, developmental disabilities, and cancers. Their information can help all family members lower their risk and possibly detect diseases early.
  • If your relatives have had cancer, note the age at which they were diagnosed. Cancers diagnosed at a younger age have a higher risk of being hereditary.
