You can request copies of your medical records by submitting an authorization form (based on the facility from which you are requesting records) to the Mass General Brigham Medical Records Department. View the authorization forms. Health Information Management (HIM) can provide you with copies of your medical records related to care and treatment you received at Newton-Wellesley Hospital. Please keep in mind that HIM does not provide copies of hospital bills or radiology films. For radiology films, call 617-243-6071. For copies of hospital bills, call 617-243-6100.
