Many health insurance plans cover nutrition visits related to medical problems. We encourage you to call your insurance carrier directly before your pediatric nutrition counseling appointment to make sure your visit is covered.
Please plan on arriving 10 minutes before your child’s scheduled appointment. If you can’t keep your child’s appointment, we would appreciate it if you would cancel or reschedule with at least 24 hours’ notice.
Please complete the Pediatric Nutrition Self-Assessment (PDF) before your child’s visit. This will allow us to make the best use of the appointment time.
