Our Anticoagulation Management Services team at Newton-Wellesley Hospital provides you specialized, expert care for your anticoagulation (blood-thinning) therapy. Our mission is to provide you high-quality, safe, and effective care by monitoring and adjusting your doses of warfarin, or Coumadin, an anti-clotting medication used to treat and prevent blood clots.
Our team monitors your blood and adjusts your warfarin dose based on what is required to maintain a safe and effective range, as prescribed by your referring physician.

To visit our clinic, please have your doctor complete a referral form. A hospitalist can send a temporary referral to enroll you in the clinic at the time of your discharge, as long as your outpatient attending doctor sends a standard referral later.
