  • Cardiac Rehabilitation Program:
    A medically supervised healthy lifestyle and cardiac risk reduction program to help speed recovery from a cardiac event and improve long-term health. This exercise, education and lifestyle modification program is appropriate for individuals who have recently had a heart attack, angioplasty, heart surgery or have been diagnosed with angina. 
     
  • Nutrition Counseling at the Cadiovascular Health Center
     
  • Congestive Heart Failure Clinic:
    This Congestive Heart Failure (CHF) Clinic is for patients with congestive heart failure. Referrals come from inpatient admissions for acute onset heart failure and referrals from cardiologists and primary care physicians. This is a nurse practitioner based-clinic that offers disease management, education, exercise and long-term follow-up. The program also includes dietitians and physical therapists involved in the education and exercise components.

    When patients come to the CHF Clinic, they are evaluated by a nurse practitioner who performs a cardiopulmonary assessment. The nurse also reviews the patient’s medications and educates them on the causes of cardiomyopathy, optimal medical management and self-care. The dietitians in the programs help patients revise their eating habits by searching out low sodium choices, learning where the sodium hides in regular food (like bread) and strategizing choices in restaurants. Patients undergo a physical therapy examination with a cardiopulmonary clinical specialist, which includes a review of prior exercise habits and developing an exercise plan.
