Our Team's Role

Your Case Manager/Social Worker team is here to help you. Our primary role is to assist you and your family in developing and coordinating a discharge plan that is based on your needs, preferences, insurance benefits and recommendations from your health care team.

We can help you:

• Understand your insurance benefit including how this will impact your discharge plan.
• If you are a Medicare subscriber, provide you with the telephone number to a quality review organization in Massachusetts to help answer questions you may have about your rights, including your right to appeal the Hospital’s decision to discharge you.
• Initiate an appeal should you disagree with your readiness for discharge or the discharge plan.
